Low power photomicrograph of the surgical specimen shows vascular capillary tubes lined with plump epithelioid-looking endothelial cells with a fair amount of atypicism to establish the diagnosis of a intermediate to low-grade hemangioendothelioma. This is the type of case that frequently requires adjuvant low-grade radiation therapy along with surgical debridement of the lesion. The prognosis is quite good in these low-grade lesions and pulmonary metastases are rare, however, in some cases they will upgrade and advance into a more aggressive angiosarcoma that might require amputation.
