On this CT scan, which was probably a staging process looking for the primary disease, there is a renal cell carcinoma involving the parenchyma of the kidney on the left side opposite the liver. This patient was treated by systemic Interleukin 2, along with a nephrectomy and local radiation therapy for the pelvic lesion to avoid pathological fracture in the future. If a surgical debulking of the pelvic tumor is attempted, the surgeon should be aware of the extensive hemorrhage that can occur in this disease process and the patient should be embolized immediately before the surgical procedure. This patient would also be a good candidate for the use of Aredia, a bisphosphonate to inhibit osteoclastic activity which is quite aggressive in this type of metastasis.
