Axial T1-weighted MRI shows again the high signal features of the intramuscular lipoma in the quadriceps muscle with a serpinginous low signal pattern spread throughout, quite diagnostic of an angiolipoma. In this case, because the lesion was so large, a marginal surgical resection would have caused considerable disability and the patient was advised on a conservative program with compressive stockings to avoid swelling and possible hemorrhage into this area over the years. The chance of malignant degeneration is extremely unlikely.
