Axial T2-weighted MRI reveals the high signal features of this soft tissue tumor mass filling the entire anterior compartment of the leg that would strongly suggest the diagnosis of a soft tissue sarcoma. This patient was treated by below-knee amputation because local infiltration of the tumor made a local wide resection virtually impossible. The patient died four years later from metastatic disease.
