Because of pain and arthritic changes associated with longstanding PVNS, this patient was taken to the operating room. This was before the days of total hip replacement so a Smith-Peterson cup arthroplasty was performed as seen on this postoperative radiograph. Nowadays this patient would have been treated by a routine total hip arthroplasty at which time the lytic area in the supra-acetabular area would have been packed with either methyl methacrylate or bone graft material.
