A 17-year-old female presented with a history of chronic, painful swelling about the ankle. This T1-weighted MRI shows a low signal soft tissue abnormality, both anterior and posterior to the ankle joint, with extensive involvement above the ankle joint posteriorly and down below the subtalar joint that strongly suggests a synovial and tenosynovial etiology. This picture is characteristic of PVNS with associated tenosynovial involvement of the tendon sheathes running by the ankle joint.
