A 17-year-old boy presented with symptoms of dull, aching pain in his distal thigh for about six months. This AP radiograph of the distal femur shows a chronic, granulomatous, lytic process within the distal diaphyseal-metaphyseal portion of the femur with reactive periosteal new bone formation on the medial side that suggests chronic, low-grade inflammatory disease, such as infection, eosinophilic granuloma, or perhaps a Ewing's sarcoma. In this case, the biopsy proved the diagnosis of eosinophilic granuloma.
