Coronal STIR MRI study of the distal femur shows a bright high signal abnormality involving most of the distal femoral medullary canal as well as high signal periosteal changes at the periphery suggesting inflammatory disease or perhaps Ewing's sarcoma. The patient was taken to the operating room for a diagnostic biopsy at which time the granulation tissue curetted from the medullary canal was examined histologically.
