A 2-year-old boy had dull, aching pain in his thigh for three months. This AP radiograph of the femur shows a centrally located, lytic process in the mid-diaphysis of the femur with reactive periosteal new bone formation in an onionskin pattern that might suggest the diagnosis of a Ewing's sarcoma or osteomyelitis. A biopsy was carried out at the time of curettage of the lesion and the diagnosis of eosinophilic granuloma was established. The patient required no further treatment.
