A 8-year-old boy presented with aching pain in his thigh of past six months duration. This AP radiograph of the femur shows granulomatous lytic changes throughout the mid-diaphysis of the femur with reactive periosteal new bone formation that would be compatible with the diagnosis of osteoblastoma, osteomyelitis or perhaps a Ewing's sarcoma. At the time of curettage, the histological diagnosis turned out to be eosinophilic granuloma. Following curettage, the patient's pain subsided and there was no recurrence following this simple surgical treatment.
