A 3-year-old boy presented with clinical symptoms of pain and swelling in the right thigh area for four months. This AP radiograph shows chronic granulomatous changes throughout the entire mid-portion of the femur with adjacent dilatation of the femoral cortex secondary to reactive new bone formation. This picture could suggest the diagnosis of chronic osteomyelitis, metastatic neuroblastoma, osteoblastoma or, in this case, eosinophilic granuloma.
