A 6-year-old boy presented with symptoms of pain in his shoulder for the past seven months. This AP radiograph of the shoulder shows a diffuse lytic process of the upper metaphyseal-diaphyseal portion of the humerus with a thinned outs and dilated cortex and a multiloculated appearance that would suggest the diagnosis of fibrous dysplasia or a solitary bone cyst. At the time of curettage of the lesion, the diagnosis proved to be eosinophilic granuloma and the lesion went on to heal spontaneously without further difficulty.
