A 2-year-old boy presented with enlarged liver and spleen, anemia, lymphadenopathy and multiple lytic lesions of the entire skeletal system. This lateral radiograph of the skull shows geographic, lytic lesions of the entire calvarium with sharp margination, suggesting the diagnosis of Hand-Schüller-Christian disease or Letterer-Siwe disease. This patient had extensive involvement of soft tissue viscera and thus the diagnosis of Letterer-Siwe disease was established based the biopsy of a femoral lesion.
