This lateral radiograph of the skull of a 2-year-old girl shows extensive lytic destruction of the entire calvarium with a geographic margin suggesting a generalized histiocytosis, such as Hand-Schüller-Christian disease or Letterer-Siwe disease. This patient also had involvement of the viscera, skin, otitis media, enlarged liver and spleen, and lymphadenopathy with a generalized bone marrow depression. In spite of the use of chemotherapy, the patient died six months after the diagnosis of Letterer-Siwe disease.
