An 11-year-old girl presented with a slight bulge in her forefoot. This AP radiograph of the foot shows a geographic lytic process involving the midportion of the first metatarsal with thinning and slight dilatation of the cortex and a ground glass appearance to the central lytic area, very characteristic for the diagnosis of monostotic fibrous dysplasia. In this case, surgical treatment was not necessary.
