This sagittal T1-weighted MRI reveals the low signal features of the MFH arising from the inferior pole of the patella and filling the entire anterior portion of the knee joint down to the tibial menisci. This patient was taken to the operating room and wide resection of the entire patella and extensor mechanism was carried out, followed by reconstruction with a fascia lata graft and local radiation therapy. The patient had no local recurrence and was alive and well four years later.
