A five-year-old boy presented with inflammatory pain about the distal aspect of his leg. AP radiograph of the leg shows evidence of a permeative lytic process involving the mid diaphysis of the fibula with periosteal new bone formation that could represent the diagnosis of osteomyelitis, Ewing’s sarcoma or eosinophilic granuloma. Biopsy proved the diagnosis of eosinophilic granuloma. This was treated with curettage and the disease resolved spontaneously.
