A 40-year-old male presented with a history of chronic low back pain for one year. This AP radiograph of the lumbosacral spine shows vague osteopenia of the entire axial skeleton. This patient had an elevated serum alkaline phosphatase level and an elevated IGG kappa immunoglobulin level on serum electrophorectic studies. A biopsy of the lumbar spine revealed a picture similar to osteomalacia with incomplete mineralization of the bone with wide, poorly mineralized osteoid seams, similar to what is seen with hyperparathyroidism. In this case, the parathormone levels were normal.
