A 91-year-old female presented with symptoms of pain in the sternum and an obvious palpable mass just beneath the skin that was felt to be a possible metastatic lesion from either the thyroid or kidney. This axial CT scan shows the aneurysmal appearance of the lesion in the midportion of the sternum. Reddish brown material was found early in an open biopsy procedure. At one point the curette dropped into a very large aortic aneurysm that presented large amounts of pulsatile arterial blood that almost caused the patient to die of hemorrhagic shock on the table. Fortunately the surgeons were able to block the defect with methyl methacrylate and close the skin. No tumor was found in the tissue sent to pathology.
